PRP

Platelets are part of blood like white and red blood cells. The platelets don't have nucleus, so, they are not called cell. The size of platelets is smaller than white and red blood cells some what. when bleeding occur because of vascular injury, the platelets become active quickly and prevent bleeding by blood clots.
The platelets create blood clots by two mechanisms: 1. the necessary chemical factor for blood clot creation, 2. transformation and interconnection. Platelets activities is not only creating clot, but also each platelet is a biochemical source in the recovery factors and tissue rehabilitation in injuries.

What are Platelets and How is made?

PRP is platelets rich plasma which is provided of patient's blood. To obtaining PRP, the patient's blood get out of the body by personnel with a syringe containing Citrate and centrifuges, then red, white blood cells and platelets are separated in different levels in test tube and finally, Platelets Rich Plasma is removed from the appropriate level which contains 4 to 8 times platelets in each centimeter.

PRP in Hair Implantation

PRP application in hair implantation:
1 . To maintain and strengthen hair follicles and new hair growth
2 . In order to promote healing and tissue repair after hair implantation
3 . To stimulate dormant hair follicles growth and new hair growth

Follicles can damage between the time that hair is removed from harvest area to transplant area in various cases:
- decreasing hair texture 's water (between the time which is taken to implantation )
- decreasing nutrients and Oxygen due to lack of blood flow during harvest time to transplant
- disorienting hair unit temperature and alkali-acid from harvest to transplant stage
- disorienting of blood flow in removed unit to adapt re perfusion in implantation area

PRP Complications

The utilization of PRP and follicle persistence in hair implantation:
Sometimes, it may seem strange that how the hair follicles can be transferred from back of the head to in front of the head and stay alive? The factors that cause to damage in implanted tissue are:
1. Trauma to the follicles
2. Biochemical factors
3. Vascular factors(Oxygenation, blood supply)

trauma to the follicle by cutting, isolated follicle dehydration, primary units crushing and change them to follicle units occur during implantation surgery which of course, in crushing tissue to units, using a microscope and keep moist tissue, also, accuracy in implantation minimize tissue destruction which caused by it.

Biochemical factors have pointed to this subject that when hair unit is separated from the primary site, cause to cut off blood supply, environmental reserves and temperature change and PH tissue is disrupted and follicle cause to Oxygen and necessary nutrient deficiency, also all these can cause tissue Schemia and accelerate destruction. Therefore, most of the physicians use Normalsalin for tissue maintainance from harvest time to implant to reduce the resultant damage.

Vascular factors which are contained rapid Oxigenation after surgery and availability of new vessels in tissue are other factors for the survival of implanted tissue. Each organ which is implanted, it's supplying tissues are implanted too. Except to hair follicles in which hairy vessels in each follicle does not exist and so called, the grafts are free.

To achieve follicles to Oxygen, there are proposed methods which is topical use of Hyperbor Oxygen or vascular bronchodilators can be used to achieve better oxygen. Also, can be used for vascular growth factors that are easily accessible. PRP has been recognized since 1970, but Recently has been used as a factor in the treatment process complication. Currently, PRP is used extensively in Orthopedic surgery, dentistry, maxillofacial surgery. Considering that the entire process is done with strilization and in disposable devices, the possibility of contamination is closed to zero.

How PRP help to Hair Implantation?

Remember that the body platelets are the main factor in the healing of the scar, when a scar is created by incision the clots besides the scar are containing platelets which start to recovery process. Platelets growth factor stimulate formation of new blood vessels and collagen production to scar healing to be done. Moreover, cause to the existed bacteria around the scar disappear. The important thing is that PRP should be condensed to the extent that has therapeutic benefits, in the current system, PRP platelet is equivalent to 5 times plethora in the coronary blood flow (is defined for 10cc PRP).

UEBEL and GRECO are researched at the same results in using PRP to achieve early recovery and reduce complications such as redness and destruction of follicles after implantation. Following this interpretation, why the use of PRP is not common? Due to high cost of equipments such as centrifuge and disposable devices. PRP is a new method which many surgeons are not familiar with it.
